A separate reciept is provided for each patient, regardless of the name that the actual account is held in, ie mother orders for son. Health Funds require a separate reciept for each patient, it’s important that the patient name matches the health fund member name for easy claiming.

Prescription Date:*

Your contact lens prescription should not be more than two years old. When you insert your prescription date it allows us to remind you to see your Optometrist prior to ordering so your prescription remains valid.
